Both became professors at the Johns Hopkins University School of Medicine, Sam of neurosurgery and Sarah of pharmacology, Eventually, the pair saved enough money for their own yacht, a Hatteras.

Without Remorse

In the fall of 1970, Sam and his wife has been traveling on their yacht in Chesapeake Bay when they struck a sandbar that was not on their outdated nautical chart. To make matters worse, Sam had neglected to replace the zinc anodes that normally protect exposed metal from corrosion, resulting in the yacht's loss of it's propellers.

John Kelly, who happened to be nearby, towed their boat to his home on Battery Island. Kelly ordered new parts for them as well as an updated nautical chart for them. While waiting for parts, Sam and his wife stayed on the island over the weekend. During this time, Sam became good friends with Kelly while Sarah bonded with Kelly's girlfriend, Pam Madden. While unloading Kelly's boat one day, Kelly accidently dropped Pam's bag on the ground, spilling its contents. Sam and Kelly learned that Pam had a barbiturate addiction and later confronted her about it. In return for helping them with their boat, Sam and Sarah helped Pam get started on her addiction recovery.

Two weeks later, Sam was on shift at John Hopkins University when Kelly was brought in after suffering serious injuries by a shotgun from Billy Grayson. Sam initially did not recognize Kelly but noticed the tattoo on his arm which he remembered while staying on Battery Island. Sam then personally performed the surgery to remove the hundreds of pellets lodged in Kelly's body with the help of nurse Sandra O'Toole. Afterwards, Sam spoke with Kelly and learned that Pam had been taken and was possibly dead. Sam then spoke with several police officers and told them of his time at Battery Island with Kelly and Pam. Sam later ordered another doctor named Anne Pretlow not to tell Kelly he had contracted gonerrhea from Pam as he did not want that to be what Kelly remembered her for. Instead he had Pretlow tell Kelly the treatment pill was for a "post-op infection". A short time later, Sam broke the news to Kelly that Pam's body had been found.

San tried to convince Kelly that her death was not his fault but was unsuccessful. Sam then tried his best to keep detective Tom Douglas from questioning Kelly too hard as he did not want to risk stressing him out further. He then convinced Douglas to allow him to stand in during the interview. After Douglas introduced himself to Kelly, he tried showing him the photo of Pam's body to identify her. Enraged, Sam tried to stop Kelly from seeing the photo and pinned Douglas against the wall. The photo landed in Kelly's lap, however, causing him break down in shock. Douglas tried to apologize but Sam continued to reprimand him, stating the he could of identified Pam and that shock like that could kill someone. Despite this, Kelly collected himself and told Sam it was alright as Douglas was only doing his job.

Red Rabbit

In 1981, Sam was approached by his colleague, Dr. Cathy Ryan, who asked him to examine her husband Jack Ryan. This was because Jack had been suffering from chronic pain in his neck and back as a side effect of wounds sustain from a helicopter crashing healing improperly. Sam later operated on Ryan's back and cured his chronic pain in relatively short order.
